III. This contract is entered into on the _______ day of _________, 20___ by The Serpent
Sanctuary and ________________________________________ (adopter) Hereinafter “I” will refer to the adopter and “animal” will refer to the animal described above.
Adoption Agreement:
(By initialing each item you indicate you have read, understood and agreed to the conditions listed.)
1. I agree that the animal is being adopted for myself and will not be sold, adopted or given to another party. ______
2. I agree to care for the animal in a humane manner and be a responsible animal guardian. This includes providing proper food, water, shelter, attention and medical care. _______
3. I agree to provide adequate UVB light and heat to keep the animal healthy. ______
4. I agree that if at any point I cannot keep the animal, I will either return the said animal to The Serpent Sanctuary without compensation. ______
5. I understand The Serpent Sanctuary makes no guarantees about the animal’s temperament and is not responsible for further damages or injuries caused by the animal. _______
6. I agree the animal will not be used for medical experimentation. _______
7. I agree that The Serpent Sanctuary reserves the right to follow up on the welfare status of the animal including but not limited to an in home visit at any reasonable time and if animal is not adequately cared for The Serpent Sanctuary reserves the right to remove the animal from my possession. _______
8. I agree to take the animal to a licensed reptile and amphibian veterinarian within 60 days of adoption for a physical exam and if the animal requires medical treatment I understand I am fully responsible for the medical expenses. _______
9. I agree to follow the ordinances and regulations set forth by my local government and obtain any required permits or permission to house said animal. _______
10. I understand if at anytime I violate this contract in any way I will pay a $200 penalty, in addition to all legal fees and return said animal back to The Serpent Sanctuary. _______
11. I agree to pay the sum of $________ to the Serpent Sanctuary as an adoption fee. _______
IV. To the best of The Serpent Sanctuary’s knowledge, the physical/medical condition of the animal at the time of adoption is healthy unless noted below:
By initialing, I understand that any physical and potential medical conditions listed above are my responsibility and agree to pay for any medical expenses incurred. _______
V. I hereby agree to these terms and conditions and understand this is a legally binding contract.
